Apply, Renew, Replace

Follow the instructions below to do any of the following:

  • Apply for a new permit  
  • Renew an existing permit (up to 3 months in advance)
  • Replace a lost/stolen permit

  1. Download Application for Disabled Person Parking Permit (PDF)
  2. Complete and sign your portion
  3. Provide the form to your medical professional and ask them to complete and sign their portion.
  4. Once both portions of the application have been completed, return it to:
